Owner Relinquish Form

Our Policy

At Illinois Cocker Rescue, we feel it is extremely important for Cockers to remain in their original homes if at all possible. To facilitate this, we spend time with owners trying to help them resolve their Cocker's problems. For certain behavioral issues, we recommend obedience classes and other training methods; for temperament or aggression issues, we recommend working with an animal behaviorist and/or having the Cocker's thyroid tested; for health issues such as allergies, ear infections, or skin ailments, we recommend feeding a natural diet; and for allergies to their Cocker, we also recommend that the Cocker be put on a natural diet.   

However, when all else fails, certain circumstances dictate that an owner must relinquish the Cocker. The following are ICR's requirements to accepting these dogs:

1.  Please fill out and submit our Owner Relinquish form.

2.  After submitting the Owner Relinquish form, you will see a page with a synopsis of your answers.  Please copy and paste the synopsis onto an email to IL Cocker Rescue, together with a recent (scanned) photo of the Cocker you are relinquishing.

3.  If we accept your Cocker into our program, we will schedule a time for you to bring the Cocker to our facility and provide directions.

4.  At the time a Cocker is relinquished to our facility, we require

    (a) a minimum donation of $100 for each Cocker relinquished;

    (b) a copy of all current vet records including proof of spay/neuter;

    (c) that all AKC records must be signed over to Illinois Cocker Rescue; and

    (d) that owners execute a legal document officially relinquishing their dog to Illinois Cocker Rescue.
